Curonix LLC and its subsidiaries (“Curonix” or “we”) respects your privacy and we are committed to protecting your privacy. Curonix LLC is a US company with a principal office at 1310 Park Central Blvd S, Pompano Beach, Florida, 33064, USA. This policy describes the types of information we may collect from you or that you may provide when you:

  • supply services to Curonix;
  • provide your resume or CV to Curonix or apply for a role at Curonix
  • engage with Curonix in your capacity as a Healthcare Professional (“HCP”); and
  • engage with Curonix as a patient.

It covers our practices for collecting, using, maintaining, protecting, and disclosing that information.

This policy applies to information we collect:

  • on this Website;
  • when you provide your resume or CV to us or apply for a role at Curonix;
  • when you use the Curonix Compliance Hotline (please note that our hotline can be used on an anonymous basis but Curonix is unable to prevent personal data being shared where an individual chooses to share it);
  • in email and interactions between you and this Website;
  • when we communicate with you in your capacity as a HCP;
  • when we receive services from you as a supplier, agent or distributor; and/ or
  • when we engage with you as a patient.

It does not apply to information collected by:

  • us through any other means, including on any other website operated by Curonix or any third party (including our affiliates and subsidiaries); or
  • any third party , including through any application or content (including advertising) that may link to or be accessible from or through the Website.

2. Children Under the Age of 18

Our Website is not intended for children under 18 years of age. No one under age 18 may provide any information to or on the Website. We collect several types of information from and about users of our Website, patients, suppliers, candidates for employment, users of our compliance hotline, and HCPs including information:

  • by which you may be personally identified, such as name, postal address, email address, telephone number, or health information (“personal information”);
  • about your role within the organisation you work for (if you are a supplier);
  • about your qualifications, from references from your previous employers, previous work experience, right to work in certain locations (if you are a candidate for employment);
  • that is about you but individually does not identify you;
  • about your internet connection, the equipment you use to access our Website, and usage details; and/ or
  • about your role as a HCP.

We collect this information:

  • Directly from you when you provide it to us (including when you use our compliance hotline) in the context of industry events; and
  • Automatically as you navigate through the Website. Information collected automatically may include usage details, IP addresses, and information collected through cookies and other tracking technologies.

4. Information You Provide to Us

The information we collect directly from you in your capacity as HCP, patient, candidate for employment, compliance hotline user, or supplier may include:

  • Contact information including your name, e-mail address, phone number, home address and business/ workplace address;
  • Information related to use of our devices;
  • Complaints about our devices;
  • Information in relation to your role;
  • Information relating to your previous experience, qualifications, and right to work in certain locations; and
  • any personal information you choose to share, including when using the compliance hotline.

The information we collect on or through our Website may include:

  • Information that you provide by filling in forms on our Website. This includes completing a “contact us” form, or requesting further services. We may also ask you for information when you report a problem with our Website.
  • Records and copies of your correspondence (including email addresses), if you contact us.

5. Information We Collect Relating to Patients Via HCPs or Medical Facilities

  • Unique Patient number i.e. used as part of medical records in your country. One example is the NHS Community Health Index number used in the UK. We typically use the last four digits of this number.
  • Unique patient identification number used by the medical facility; and
  • the serial number of the device that is implanted.
  • Any other patient information received from HCPs or medical facilities;

6. How We Use Your Information and Our Lawful Basis for Processing

We must have a lawful basis to process your personal information. When processing your personal information for the purposes set out below, we will rely on one of the following lawful bases:

  • Our legitimate interests to deliver our products and services and grow our business.
  • Our legitimate interest to review and manage applications for jobs at Curonix.
  • Our legitimate interests to improve our products and services.
  • To fulfill our obligations to patients.
  • To fulfill our obligations to HCPs and suppliers.
  • Our legitimate interest to ensure that our Website operates effectively and is secure.
  • Our legitimate interest to contact you where you provide us with your details to manage our relationship with you and to provide you with information about our products and services.
  • Our compliance with a legal obligation where we are required to collect and disclose your personal information because we are subject to a legal or regulatory obligation.

Where we process health data of patients we either collect explicit consent from patients or rely on the condition that processing is necessary for the provision of health care or treatment.

We use information that we collect about you or that you provide to us, including any personal information for the following purposes:

  • To present our Website and its contents to you.
  • To provide you with information, products, or services that you request from us or which your HCP or medical facility requests in consultation with you.
  • To monitor the effectiveness of our products or services.
  • To carry out our obligations and enforce our rights arising from any contracts entered into between you and us, including for billing and collection.

  • To allow you to participate in interactive features on our Website.

We may disclose aggregated and anonymous information about our users, and information that does not identify any individual, without restriction.

Due to the global nature of our business, your personal information will be disclosed to Curonix entities and third parties we engage outside of the United Kingdom (“UK”) or the European Union (“EU”)/ the European Economic Area (“EEA”), including in the United States.

It may also be processed by Curonix employees or suppliers operating outside the UK or EEA.

We provide protection for the transfer of personal information to countries outside of the UK or the EU/ the EEA, including by (i) relying on a country-wide adequacy decision (where a country offers substantially the same protection as UK and/or EU data protection laws, as applicable) and (ii) the inclusion of contractual protections in our agreements with entities based outside of the UK and/ or the EU/ the EEA.

  • Request that your personal information be provided to you or a third party.
